MAIN DUTIES AND RESPONSIBILITIES

  • Must be able to explain to clients the principles of Stop Smoking Services so that they can make an informed choice to stop smoking.
  • Will motivate and empower clients to make behaviour changes.
  • Must have a clear understanding of the need of outreach services and their impact on those living in disadvantaged areas.

Are you are looking for a new and exciting challenge? We need Care Coordinators in the Basingstoke CMHT to support the delivery of high quality community mental health services in the North Mid Hampshire area.

The team has a strong focus on patient and carer engagement, providing excellent personalized care for our patients and staff wellbeing. We are urgently recruiting for band 6 Care Co coordinators to join our team. The post-holder is expected to fill a care co-ordination role.

The post holder will operate within the principles of recovery, deliver a high standard of care, developing care plans, risk assessments and crisis plans with service users and their family, enabling a planned integration into local community facilities and then discharge as part of the patient’s recovery plan.

As an experienced mental health practitioner, the post holder will direct and supervise junior staff and/or students and resources within their area of responsibility and take an active participation in clinical quality assurance and learning programmes. Clozapine & Depot clinics, outreach depots, co facilitate groups with psychology colleagues.

  • To have clinical responsibility for a case load as specified by the Team leaders
  • To assess service user needs, plan effectively, monitor and evaluate interventions using evidence-based practice which is relevant to individual needs and within a framework of research and audit.
  • To participate in duty work on a rota basis with other team members when necessary.
  • To support service users to comply with medication regimes, and where appropriate to administer depot medication.
  • To provide interventions in the most appropriate settings according to service user need and choice, including client’s own homes, community venues, and CMHT premises.
  • To provide the necessary supervision of other professionals and students where applicable.
  • To develop effective partnerships within the multidisciplinary team across other agencies.
  • To ensure the implementation of Trust and national policies in particular with regard to CPA and risk strategies.
  • To maintain comprehensive case records in line with Trust requirements and using RiO, the Trusts electronic record system.
  • To maintain a holistic approach with clients and work with them in all aspects of their lives in a variety of settings, in order to promote health and maximise independence.
  • To be aware of the professional responsibilities in relation to the UKCC Code of Practice and Nursing and Midwifery Council guidelines.

As a Vending Machine Stock Replenishment Representative, you will form part of our Customer solutions team and will play a crucial part in our Large Customer operation. You will understand our customer’s current vending implementation experience and make recommendations to improve their offering. You will also replenish vending solutions onsite for our customers whilst developing great relationships to become a trusted partner.

You will also be a great support contact for our customers. Through collaborating with our implementation team, you will feel confident to support our customers in the event of any vending machine faults. Faults are typically resolved by using the computer, so you’ll need to ensure you have a good understanding of IT to truly add value to our customers. Your can-do attitude and willingness to help our customers will ensure the maintenance of their equipment and that their operation will continue to run smoothly or with as little delay as possible.

You will also be joining our team with a fresh perspective and you can make a real impact by asking questions about the way we do things. You will help us to continuously improve our business and we will value and consider everything that you say.

In a Nutshell:

As our Customer Service Co-ordinator, you will deal with customer concerns received by telephone or written communication and co-ordinate and administer all activities in a timely and professional manner, delivering the Vistry commitment to the customer journey.

 

What we would like from you:

Key Responsibilities –

  • Keep purchaser’s plot file information up to date.
  • Keep purchasers, Customer Service Manager, and Subcontractors up to date at all times as appropriate.
  • Record receipt of all purchaser’s correspondence and respond within 3 working days.
  • Record receipt of all purchaser’s telephone calls and return a courtesy call on the same day.
  • Issue instructions to Sub-contractors.
  • Monitor and update NHBC claims if appropriate.
  • Assist cost monitoring.
  • Follow up all outstanding remedial works and take appropriate action to ensure works wherever possible are completed within a 28 day period.
  • Record dates of customer visits and log all defects identified.
  • Carry out all necessary calls in relation to the customer journey.
  • Keep data system up to date at all times.
  • Carry out general administrative duties.
  • Work directly with Customer Service Manager to assist management of large scale works.

Competencies –

  • 5 GCSE’s or equivalent including Maths & English
  • Previous experience working within a similar environment
  • Good understanding of Microsoft Office, Excel, Outlook
  • Ability to handle complaints and difficult situations
  • Good planning and organisations skills
  • Problem solving and decision-making skill
  • A polite, tactful and assertive attitude
  • Patience and calmness under pressure
  • Excellent communications skills
  • Good team working skills
  • Behave in line with our values
  • A commitment to work as required to meet the needs of the business

Desirable:

  • NVQ levels 3 & 4 in customer services
  • Experience working for a residential house builder ideally within the customer facing environment
