Position Description:
The Mercy Health Medical Center is looking for experienced Clinical Nurse Specialists only. The Clinical Nurse Specialist will be hired to work at the Mercy Health Medical Center when Penn Medicine and the Hospital of the University of Pennsylvania assume nursing operations at the end of March. You can expect to begin your training on units at the Hospital of the University of Pennsylvania prior to the moving over to Mercy Health Medical Center.
The Clinical Nurse Specialist (CNS) role is a clinical leadership position. The CNS serve as expert clinicians within a specific patient population and partner with Nurse Managers to assume accountability for the nurse and patient outcomes on their nursing unit. Leveraging advanced education and training, CNSs are accountable for leading nursing practice by supporting nurses to care for patients at the bedside, driving practice changes at the unit, service line, and systems levels, and ensuring the use of best practices and evidence-based care to achieve the best possible patient outcomes. An important focus of the CNS role is to understand organizational goals and design, implement, and evaluate practice change initiatives that advance these goals.
A strong CNS must be able to: (a) communicate effectively toward negotiating multiple perspectives within the organization and driving change; (b) demonstrate clinical judgment and establish clinical credibility with nursing and interprofessional colleagues within his/her specialty area; (c) exhibit self-direction in maintaining professional knowledge and competence within his/her specialty area; (d) apply critical thinking and analytic skills to design and implement change initiatives; (e) build rapport with and provide mentorship to clinical nurses; and (f) partner effectively with nurse manager colleagues to advance professional nursing practice.

Minimum Requirements:
-Master's Degree in Nursing
-5+ years Experience in clinical practice with the patient population cared for in the unit/ area

As part of the Quality Management team, the Quality Performance Specialist is responsible for leading and coordinating the clinical quality review process of member concerns and medical records utilizing of care standards. In addition, responsibilities also include supporting the annual medical record review activities including HEDIS and Clinical Practice Guidelines. The position also supports accreditation and clinical quality performance improvement activities.

  • Assumes responsibility for reviewing all sentinel events and member concerns reported for potential quality of care issues.  Coordinates reviews with physician advisors and clinical quality committees as appropriate. 
  • Assures minimally necessary medical records are collected and organized for clinical review to occur. Summarizes all clinical findings for review by clinical committees as appropriate.
  • Prepares meeting packets, schedules meetings, develops agendas, completes minutes, and chairs the Internal Quality Review Committee (IQRC).
  • Maintains database to support trending and reporting of adverse outcomes resulting from clinical care provided. 
  • Develops reports and analysis for semi-annual trending reports and presents to the Quality Medical Management Committee (QMMC) for input and direction.  Follows up on any recommendations.
  • Maintains QR policies including annual reviews, updates, and approvals.
  • Maintains hard copy documentation in accordance with contractual requirements. 
  • Conducts analysis annually on the Plan’s efforts to improve the safety of members.
  • Supports the quality improvement department with annual medical record review activities including HEDIS, PA Community Health Choice, and Clinical Practice Guidelines by working collaboratively with the Quality Improvement Department.  Assistance will be based upon the organizations needs annually but may include participation in the development of medical record review tools, conducting training and testing of medical record review staff, and auditing medical record reviews upon completion.   

Education/Experience:

  • Current and unrestricted Registered Nurse licensure in Pennsylvania.
  • Clinical experience in home care and/or case management experience.
  • 3 to 5 years quality improvement experience required
  • 2 to 4 years’ experience in project facilitation/management role demonstrated by leading teams on initiatives related to wellness and quality measures.
  • 3  to 5 years’ experience within managed care (Medicaid/Medicare) in a clinical capacity strongly preferred.
  • Knowledge and understanding of quality improvement functions within a managed care organization preferred.
  • Strong analytical skills.
  • Proficiency utilizing Microsoft Office applications including Word, Excel, and PowerPoint required.

DaVita is seeking a Registered Nurse who is looking to give life in an outpatient dialysis center.  You can make an exceptional difference in the lives of our patients and their families dealing with end-stage renal failure or chronic kidney disease.

If you haven’t considered Nephrology before, read on as we think that you should. Dialysis Experience is NOT required.

What you can expect:

  • Empower your patients. As both a nurse and educator, you will teach your patients about dialysis and enable them to take their health back into their own hands. Training patients takes time and mental energy and the rewards of that investment come in seeing the transformation of your patients from feeling sick and afraid, to strong and independent.  
  • A team that feels and functions like a family. Staff RNs are a central part of a small interdisciplinary team of clinicians. Work alongside and support technicians, other nurses, dietitians, social workers, physicians and the center manager.  
  • Fun is one of our core values.  Happier nurses = healthier patients.
  • Specialized, complex care. RNs deliver care to patients who are often dealing with multiple co-morbidities which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge.  
  • Fast paced. Our RNs oversee a group of patients (census dependent on state laws) with the support of PCTs to conduct patient observations, measuring stats and machine set up.  You will work with your head, heart and hands each day.
  • Schedule. Shifts are typically 10-12 hours. The schedule for this role will be fours days a week (8am to 6pm) or three days a week (7am to 7pm), dependent on preference. Weekends and early mornings are NOT required.
