Position Overview:
The Clinical Pharmacist
will work independently and in consultation with physicians and other health
care providers across the health care continuum to ensure appropriate
medication therapy for individual patients within the community based on
outpatient practice environment.
This
will be accomplished using patient interviews and counseling, reviewing the
medical record and medication profile, consulting with physicians, teaching,
staff education, medication use evaluation, adverse reaction monitoring and resolving
medication-related problems. The
clinical pharmacist's interventions will target this high risk population
through the provision of medication reconciliation and comprehensive medication
use management in focused disease states.
In addition, the clinical pharmacist
will also assist with care coordination, disease state education and monitoring
to achieve optimal outcomes. The
Clinical Pharmacist will be focused on chronic disease state management as part
of a coordinated team. The Clinical Pharmacist
must be capable of using his/her professional judgment and clinical expertise
in the daily solving of complex problems and the in the provision of clinical
information within the scope of practice.
Qualifications:

+ Doctor of Pharmacy (Pharm D) degree from an accredited college of pharmacy or equivalent work and practice experience

+ Minimum if two years of pharmacy experience with background in preparation of IV admixture including chemotherapy

Position Summary:

 

Pharmacist Associates ensure timely and accurate preparation and distribution of pharmaceutical products within Dominican Hospital. This position assures that the medication orders are consistent with current medical practice, F.D.A. and drug manufacture recommendations, current literature and/or other information sources. The Pharmacist Associate resolves variations from the above standards with members of the Medical Staff in accordance with established policies and procedures. Pharmacist Associates provide information and guidance in the proper administration of medications as necessary for health care providers, patients, and family.

  • Two years work experience in health-systems pharmacy or educational equivalent.

SUMMARY

  • Ensure the accuracy and appropriateness of all prescriptions filled by completing Drug Utilization Review and Final Quality Assurance, applicable to state and federal Board of Pharmacy regulations.
  • Build profitable business and script growth through recommended clinical programs including appropriate immunization and when available MTM (Medication Therapy Management), DCS (Diabetes Care Specialist), and all ongoing other programs as identified.
  • Provide excellent customer service by assisting customers with medical-related issues and providing healthcare counseling.
  • Supervise the work completed by support staff while on duty.
  • Assist with training, coaching and managing the pharmacy associates and assume management responsibility over the entire pharmacy as needed.
  • Maintain appropriate security of the Pharmacy department.
  • Comply with HIPAA and all other applicable federal and state laws and regulations.
  • Handle and resolve customer issues, complaints and questions to build customer trust and loyalty.
  • Interact with physicians and utilize reference material to gain information on customers and prescriptions and to resolve any issues that arise.
  • Assist with maintaining the Pharmacy department by keeping it clean and in order.
