As a Patient Care Team Nurse, you will continuously evaluate patient care systems, collaborate with the healthcare team to ensure continuity of patient care, and report patient progress. This position will work closely with the medical staff and will report to the Site Operations Director.

JOB RESPONSIBILITIES:

  • Participates in continuous quality improvement activities by evaluating patient care systems that may include standards, protocols, and documentation.
  • Collaborates with the healthcare team to ensure continuity of patient care throughout the healthcare settings.
  • Promotes effective communication among healthcare team members including patient, family and significant others.
  • Performs assessment, data collection, obtain, review and analyze information in collaboration with the healthcare team members and others as appropriate.
  • Conducts pre-visit planning by reviewing charts, following up on labs, and referrals as well as obtaining necessary orders from the provider.
  • Identifies patient problems, physiological and psychological changes, deviations from normal, and identifies inter-relationships of problems, reports changes to health care providers
  • Keeps care coordination records up to date
  • Initiates, implements, and documents patient and family teaching and documents their response to teaching.
  • Identifies unsafe patient environment and intervenes responsibly. Discusses situations with Pediatricians and Nurse Practitioners for development of emergency plan for patient and family regarding their health care
  • Maintains open communication with Pediatrician, Nurse Practitioners, and other staff, reporting patient progress, abnormal diagnostic/assessment data, and notable health changes
  • Assists patients and families in functioning at their optimum level.
  • Regularly meets with Providers and other Patient Centered Medical Home team members to identify clinical problems and helps initiate appropriate solution.
  • Assists in implementing and carrying out continuous quality improvement programs and activities.
  • Coordinates services for patients and families utilizing other health related agencies and resources
  • Obtains referrals to other physicians and specialists.
  • Follows approved protocols/standing orders for patient care.
  • Ensures that a productive and efficient patient flow occurs.
  • All HealthLinc staff is committed to engage in quality improvement initiatives that align with and support Patient-Centered Medical Home (PCMH).
  • Performs other duties as assigned.

REQUIRED QUALIFICATIONS:

Education/Training

  • Bachelor of Science in Nursing (not required but highly preferred)
  • Registered Nurse licensed in the State of Indiana
  • Current CPR certification

Experience

  • At least 1-year ambulatory or public health nursing experience (not required but highly preferred)

Skills/Job Requirement

  • Strong organizational and time management skills
  • Proven ability to work well in a team environment
  • Excellent written and verbal communication skills
  • Ability to remain flexible and adaptable

The LOGAN Nurse is responsible for overseeing and monitoring the health care services for LOGAN clients and implementation of nursing support plans for assigned caseloads. Provide first aid, basic medical treatment and basic medical evaluation for individuals served and staff at the location in emergent situations; and provide staff training as directed.  As requested, the nurse will provide input into agency health, medical and safety related systems.

Requirements

Essential Functions - General

  • Performs other duties including but not limited to the following:
  • Provides medical consultation and evaluation in emergent situations for   both individuals served and staff.
  • Attends staff meetings.  
  • Ensures compliance with licensing regulations, accreditation and agency policies and procedures.
  • Provides a variety of health, medical and safety related training to staff as directed.
  • Provides input as directed into agency policy, procedure and practice related to health, medical and safety issues.
  • Provide audits of SL health and wellness processes.
  • Reviews all risk plans for effectiveness.
  • Teaches Core A and B for staff as assigned.
  • Provides CPR/First Aid Training for staff as assigned.
  • Administers medications to individuals served as directed.
  • Participates in On-Call rotation phone and responds to emergency medical issues as directed.
  • Participates on committees as assigned and performs related duties.

Essential Functions - Group Living

  • Monitors the health care of individuals including:
  • Oversees all health/medical needs as they apply to each individual served.
  • Consults with Program Managers for risk management plans and Ensures that all necessary risk management systems are in place as required by licensing regulations.
  • Ensures that all health/medical state regulations are followed. 
  • Reviews all medical health provider forms and makes copies for the Master File and Program Manager.
  • Completes nursing assessments quarterly for group home clients.
  • Ensures that all medication administration systems are in place as required by licensing regulations.
  • Ensures individual’s health is maintained at its optimal level and Ensures that all changes in an individual’s health status are promptly addressed.
  • Maintains case notes as they relate to the individual’s health and well-being.
  • Maintains a health record for individuals served.
  • Reviews risk plans, advises on implementation of risk plans to Program Managers.
  • Serves as liaison with physicians and other health care professionals including:
  • Maintains and facilitates physician and professional health care personnel contacts and communication.
  • Completes physician telephone orders, when appropriate, to validate verbal orders.
  • Maintains contact and coordinates psychiatrist services and visits to                the facility.
  • Represents Health Care Services on each individual’s support team including:
  • Reviews annual medical goals and completes as needed.
  • Attends annual support team meetings, as appropriate.
  • Oversees the medication administration system for all individuals including:
  • Reviewing medication administration data.
  • Reviewing drug count records.
  • Reviewing medication incident reports and making recommendations.
  • Coordinating monthly Medication Exchange.
  • Checking medicine cabinet and medications monthly, or more often as deemed necessary. 
  • Reviews, signs, and copies medication administration records.
  • As directed, serves as an agent to agency psychiatrist in order to give verbal orders for prescribed medication.
  • Maintains on-going relationship with pharmacy.
  • Works with consulting pharmacist to set quarterly pharmacy reviews, coordinate the review and complete follow tasks based on the identified issues.
  • Maintains pharmacy delivery receipts to verify and track medication.

Essential Functions - Supported Living

  • Provides overall monitoring of health care for assigned individuals including:
  • Oversees the consultation of all health/medical needs as they apply to each individual.
  • Oversees the consultation with staff with regards to medical care of individuals served.
  • Oversees consultations with Program Manager for risk management plans.
  • Oversees the completion of an initial annual nursing assessment of each individual and annually thereafter.
  • Ensures that individual’s health is maintained at its optimal level and Ensures that all changes in an individual’s health status are promptly addressed.
  • Maintaining case notes as they relate to the individual’s health and well-being.
  • Review of medical forms when copies provided for nurse.
  • Provide written reports, as requested, to individual’s support team.
  • Maintains primary contact with pharmacy for Supported Living medication issues.
  • Trains Supported Living staff on pharmacy procedures.
  • Assists Supported Living staff with pharmacy issues.
  • As directed, serves as an agent to agency psychiatrist in order to give verbal orders for prescribed medication.

General Responsibilities

  • Assists in promoting the health and safety of individuals served by LOGAN, visitors, and other staff.
  • Serves as role model for individuals served and other staff.
  • Serves as an advocate and protects the human rights of all individuals served at all times.
  • Participates in training as required by supervisor or for all LOGAN staff.
  • Facilitates professional growth by keeping current on new philosophy, research, technology, program strategies, etc.
  • Other duties as assigned by supervisor.  

QUALIFICATIONS

Education

  • High school diploma or equivalent required. 
  • Nurse must be licensed as a Registered Nurse or Licensed Practical Nurse in the State of Indiana.
  • Nurse must be certified in CPR and First Aid.
  • Nurse must be certified to administer TB tests.

Experience

  • Two years’ experience required.  Experience may include a combination of life experience and working with individuals with developmental disabilities

Job Knowledge

  • A positive attitude to support individuals with disabilities. 
  • A willingness to learn and implement positive approaches to instruction and behavior management. 
  • Competent computer skills. 
  • Ability to effectively communicate orally and in writing. 
  • Ability to lift a minimum of 50 pounds.
  • A valid state driver’s license in order to drive on behalf of LOGAN.

Description

As a Patient Care Team Nurse, you will continuously evaluate patient care systems, collaborate with the healthcare team to ensure continuity of patient care, and report patient progress. This position will work closely with the medical staff and will report to the Site Operations Director.

JOB RESPONSIBILITIES:

  • Participates in continuous quality improvement activities by evaluating patient care systems that may include standards, protocols, and documentation.
  • Collaborates with the healthcare team to ensure continuity of patient care throughout the healthcare settings.
  • Promotes effective communication among healthcare team members including patient, family and significant others.
  • Performs assessment, data collection, obtain, review and analyze information in collaboration with the healthcare team members and others as appropriate.
  • Conducts pre-visit planning by reviewing charts, following up on labs, and referrals as well as obtaining necessary orders from the provider.
  • Identifies patient problems, physiological and psychological changes, deviations from normal, and identifies inter-relationships of problems, reports changes to health care providers
  • Keeps care coordination records up to date
  • Initiates, implements, and documents patient and family teaching and documents their response to teaching.
  • Identifies unsafe patient environment and intervenes responsibly. Discusses situations with Pediatricians and Nurse Practitioners for development of emergency plan for patient and family regarding their health care
  • Maintains open communication with Pediatrician, Nurse Practitioners, and other staff, reporting patient progress, abnormal diagnostic/assessment data, and notable health changes
  • Assists patients and families in functioning at their optimum level.
  • Regularly meets with Providers and other Patient Centered Medical Home team members to identify clinical problems and helps initiate appropriate solution.
  • Assists in implementing and carrying out continuous quality improvement programs and activities.
  • Coordinates services for patients and families utilizing other health related agencies and resources
  • Obtains referrals to other physicians and specialists.
  • Follows approved protocols/standing orders for patient care.
  • Ensures that a productive and efficient patient flow occurs.

REQUIRED QUALIFICATIONS:

Education/Training
