Job overview

An exciting opportunity has arisen for a Pharmacy Technician to join our Primary Care Network Pharmacy Team. The post-holder will assist activities in each of the five member practices within the Rural Practice Primary Care Network to help patients to get the best from their medicines by supporting systems to improve the continuity of care, handling of prescription queries from patients and health care professionals and supporting medication management in care homes. They will also participate in medication audits, quality improvement projects and monitoring of high-risk drugs to ensure ongoing patient safety and high quality prescribing.

This post is funded by NHS England and will include a requirement to participate in the national CPPE training scheme for pharmacy technicians in general practice.

Main duties of the job

BTech. City and Guilds or Apothecary Hall, or

NVQ level 3 with BTech in Pharmaceutical Sciences or equivalent

Registered with the General Pharmaceutical Council

Practical experience of working as pharmacy technician in primary care, community or secondary care setting

Experience of providing pharmaceutical and prescribing advice

Practical experience of computerised systems

Experience of prescriptions processes and systems

Attention to detail

Good communication skills, written and oral

IT and Keyboard skills

Ability to follow procedures and work to deadlines

Ability to handle situations of potential conflict

Problem solving skills

Ability to work on own initiative

Ability to work as part of a team

Excellent interpersonal skills

Influencing and negotiating skills

Ability to plan and organise

The Rural Practice Network serves a population of 33,000 patients in South Somerset. We have five member practices - Queen Camel Medical Centre, Wincanton Health Centre, Bruton Surgery, Milborne Port Surgery and Millbrook Surgery. Job responsibilities

MAIN DUTIES AND RESPONSIBILITIES

1. To support the practice pharmacists, including:

  • To carry out medicines reconciliation in accordance with practice protocols and to update patients medical records and prescriptions.
  • To carry out medication switches, dose optimisation and medicines synchronisation to ensure safe and cost-effective prescribing
  • To initiate repeat prescribing for appropriate patients
  • To provide advice to GPs, staff and patients in changes to prescribing in order to support the improvement of prescribing safety, quality and cost effectiveness
  • To participate in medication audits and QOF quality improvement projects
  • To respond to medication queries from patients and staff in a professional manner referring to the appropriate GP in accordance with practice protocols
  • To support care homes in maintaining effective medicines management and reduce waste
  • To carry out domiciliary visits where required to support patients use of medication
  • To advise on the sourcing and safe management of medicines as appropriate
  • To action drug alerts
  • To act as a link between practice and local community and hospital pharmacy services
  • To provide information on KPIs to show benefits of the post

  • 2.To support the practice teams, including:

    • Liaise with patients and pharmacies regarding prescription queries and requests
    • Work with the practice medicines management teams, clinicians and other practice staff to resolve all medicines related queries
    • Liaise with hospital staff with reference to discharge and clinic letters
    • Check patient records for chronic disease and drug monitoring, and arrange for testing and review as appropriate
    • Enter relevant patient information accurately onto EMIS
    • Use the Prescription Tracker to establish the status of electronic prescriptions
    • Undertake any other duties appropriate to the post as requested

Qualifications

Essential

  • BTech. City and Guilds or Apothecary Hall, or
  • NVQ level 3 with BTech in Pharmaceutical Sciences or equivalent
  • Registered with the General Pharmaceutical Council

Desirable

  • Experience of systems to support patients in Primary Care setting
  • Good clinical pharmacy knowledge
  • Experience of project implementation
  • Knowledge of the General Practice environment
  • Knowledge of prescribing systems relating to GP practice and community
