Responsibilities

The RN Clinical Nurse is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality,
safe patient care. They practice in a clinical environment that is administered by Nurse Supervisors/ Office Managers and other leaders and is supported through the WellStar Clinical Champion Forum.
The framework for practice is steered by the WellStar Professional Practice Model, 5-Star Nursing and evidence based practice and research. RN competencies are derived from these models and
supported by the WellStar Core Competencies.

It is expected that all RN Clinical Nurses are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member, of the patient services team, it is expected that the individual upholds the voice of the patient, system policies and procedures while supporting service excellence goals.

Qualifications

Required Minimum Education:
Graduate of an accredited/approved school of nursing: Baccalaureate degree in nursing (BSN) from an
accredited school of nursing preferred; National certification in area of specialty preferred

Required Minimum Licensure/Certification:

• Currently licensed as a Registered Nurse in the State of Georgia or hold a privilege to practice in the State of Georgia under the Enhanced Nurse Licensure Compact (eNLC)
•  Must have a current BLS card from the American Heart Association, prior to hire date.
• ACLS/PALS may be required in some practice areas.

Required Minimum Experience:
Experience with direct patient care preferred.
Additional specialty experience may be required.

Required Minimum Skills:
Ability to read, write and speak English language, optimize the use of technology to support clinical care
and holds basic computer skills; Strong interpersonal, collaborative skills along with customer service

JOB PURPOSE:

The Home Care Administrator is responsible for organizing and directing the ongoing functions of an agency's parent or branch location. The Home Care Administrator assumes full-time administrative authority, responsibility and accountability for day-to-day operations, recruitment and retention and financial viability of the agency. This position manages partners in the provision of care and services rendered in accordance with professional standards, and in compliance with state and federal laws and regulations. The Home Care Administrator supervises Clinical Managers (CMs) and Business Office Managers (BOMs).

KEY RESPONSIBILITIES:

• Oversee all day-to-day operations.
• Ensure that a Clinical Manager is available all operating hours.
• Administrator or designee is available all operating hours.
• Ensure compliance with reporting of OASIS information.
• Ensure that agency is compliant with all aspects of patient rights, including but not limited to exercise of rights, transfer and discharge notification and investigation of complaints.
• Ensures that each patient receives a comprehensive assessment upon start of care, treatment and services with the agency. Ensure updates occur as needed.
• Ensure that care planning is developed, followed, monitored, and adjusted as needed throughout the course of care delivery.
• Ensurecoordination of services among all disciplines throughout the care of patient care delivery.
• Monitor and perform follow-up as needed for regional agencies to ensure quality of care is provided to patients.
• Serve as an active participant, guide, coach and mentor for all Quality Assurance and Performance Improvement (QAPI) initiatives necessary for agencies assigned to the region.
• Conduct QAPI with branch offices as required by regulations; liaise with branch offices and with the agency Governing board.
• Ensureall requirements are met for infection prevention and control for agencies assigned.
• Conduct job interviews, hire, and retains qualified competent personnel to manage agency operations and to provide services. Provide regular performance reviews.

MINIMUM EDUCATION REQUIRED:

• Nursing Degree from an accredited college or university, or a graduate of an accredited/approved RN program.

MINIMUM EXPERIENCE REQUIRED:

• RN with a minimum of two (2) years#39; practical experience.
• At least one (1) year of supervisory or administrative experience in home health or a related health services program.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

• Licensed physician, registered nurse or hold undergraduate degree and at least one (1) year of health service administration.
• At least one (1) year of supervisory or administrative experience in home health or a related health services program,

ADDITIONAL QUALIFICATIONS: (Preferred qualifications)

• Baccalaureate degree in nursing or healthcare administration.
• Experience in administration/ management of home health programs.

Our Neuroscience Critical Care nursing team is highly skilled with a focus on patient and family centered care. As part of this exciting team you can become a clinical expert in specialty areas such as:

• Neuro Trauma
• Cerebral resuscitation and temperature management systems
• Management of cerebral edema and increased intracranial pressure
• Hemodynamic instability
• Management of airway and respiratory compromise

• Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
• Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.

• Responsibilities of the Registered Nurse (RN)

   

  • Following the plan of care and communicating necessary revisions to the Nursing Supervisor
  • Processing orders and notifying patient’s physician of needs and changes in condition
  • Providing patient care, which requires specialized nursing skills based off patient's POC
  • Understanding and adhering to policies and procedures
  • Completing certification/recertification orders and discharge summaries
  • Observing confidentiality, safeguarding all patient related information based on HIPPA guidelines
  • Completing documentation and paperwork in a timely manner
  • Reporting any patient incidents, variances, or complaints immediately to Clinical Manager/ Nursing Supervisor
  • Demonstrating competency with technical skills, according to established procedures
